That sharp zap with ice cream is usually exposed dentin β tiny channels to the nerve that should be covered by enamel or gum. Recession from hard brushing, acidic drinks, grinding or gum inflammation leaves root surfaces bare.
Immediate relief: switch to a potassium nitrate + stannous fluoride sensitivity paste, use a soft brush with light pressure, and avoid whitening strips until reviewed. Rinse with lukewarm water after citrus or cola, and wait 30 minutes before brushing to let saliva remineralise.
In the clinic we check for cracks, leaking fillings or bite high spots that mimic sensitivity. Desensitising varnish, bonding for exposed roots or a night guard for grinding often solves it in one visit. If the zing lingers more than 15 seconds or there is spontaneous ache, we test for pulp inflammation β that needs different care.
Long-term, a six-month polish plus fluoride helps reseal tubules. Keep the paste on the teeth for two minutes before rinsing β think of it as a mini treatment, not just a scrub.
