Health claim checklist: 7 documents insurers never tell you to keep

Pre-authorisation, discharge summary, pharmacy bills β keep these seven and cashless rarely gets rejected.
Cashless rejections are almost never about whether you were ill β they are about documents arriving late, incomplete or mismatched. We handle about 40 health claims a month and this is the set that makes the difference.
The seven
- Pre-authorisation form signed by doctor and hospital TPA desk before admission (not after).
- Doctor's admission note with reason, not just "under observation".
- Investigation reports stapled to the same date β lab, scan and consultation must match.
- Break-up bills β hospital main bill plus pharmacy, implant and consumables separately.
- Discharge summary with final diagnosis and follow-up plan.
- Pharmacy bills with prescription β outside pharmacy slips without the doctor's chit get cut.
- Cancelled cheque / NEFT form for reimbursement top-ups.
Two habits that save weeks
Photo every paper before you hand it over β WhatsApp it to your advisor same day. And keep the same name spelling as on your policy across Aadhaar and hospital file; a single-letter mismatch delays TPA by 48 hours.
